Requirements:

• Adults 18 years or older
• Diagnosis by a rheumatologist of one or more of these conditions:

• Rheumatoid arthritis
• Ankylosing spondylitis
• Systemic lupus erythematosus
• Psoriatic arthritis
• Psoriasis
• Crohn's disease
• Ulcerative colitis
• Systemic sclerosis
• Mixed connective tissue disease
• Graves' disease
• Hashimoto's thyroiditis
• Sjögren's syndrome
• Atopic dermatitis
• Multiple sclerosis
• Juvenile idiopathic arthritis
• Idiopathic inflammatory myopathies (e.g. polymyositis)
• Vasculitides (e.g. ANCA-associated vasculitis)
• Other immune-mediated or inflammatory diseases as determined by study investigators

• Additional eligibility criteria reviewed during screening
